Sponsored Lines: Transfer a Line in

BYUH Students and BYUH/PCC Employees are able to sponsor friends or family members. Use this form for new lines. All transferred lines require a $30 activation fee. Note: Sponsored lines are only eligible for T-Mobile service $20/month (+taxes and fees).
This plan requires you to provide your own phone device to which we add T-Mobile service to via either a physical SIM card or an eSIM.

Included in Plan
Unlimited talk, text, and data within the united states.

Not Included in Plan
- Tax & surcharge fees are not included in the monthly price.
- International long distance fees and other miscellaneous charges are not included in the plan.
SIM Card or eSIM Activation
An email will be sent to the email address provided on this request form when the SIM card is ready for pick-up, shipped, or when the eSIM has been activated.

Added Information
- BYU–Hawaii Store is not responsible to setup your phone.
- BYU–Hawaii Store has provided helpful links on our website under FAQ.
- If your old phone is broken or lost and was not backed up to your cloud, you might not be able to get your information from your old phone.
- There is a $15.00 shipping fee to have a T-Mobile SIM card shipped. The SIM card will be shipped via Fed Ex.
Optional Added Features
- International Stateside Calling feature is optional and can be added to a line for an additional fee per month. This feature works well for those who makes international calls regularly.
- International Passes are available per carrier. More information available on our website under International Travel Options.
Non-Refundable Activation Fee
- There is a $30.00 non-refundable activation fee for each "NEW" line setup with BYU–Hawaii.
- If after submitting the request form you change your mind and decide to cancel the request, fill out THIS FORM immediately to cancel the request form. Should you try to cancel the request form after a line has already been activated for the user, there will be no refund to the activation fee.
Insurance
BYU–Hawaii Store does not provide insurance for your phone. The BYU–Hawaii Store is not responsible for your phone if it breaks or is lost.
Refunds or cancellations
The $30 activation fee is non-refundable. Plans are month to month and can be cancelled at any time. You will be charged a prorated rate depending on where in the month that you cancel your plan.
Payment
You will receive monthly invoices through the email listed above.
Late Payment Charges per line
1-14 days: $15 fee
15-44 day: Suspend Line and $30 fee to reactivate
45 days: Line is cancelled and the phone number will be lost and cannot be reactivated.

All inquiries regarding your line must be directed to the BYUH Store, the carrier will not be able to help you directly.
