Mobile Plan Acknowledgment Statement

I acknowledge and understand the following terms regarding the mobile tablet and watch plan with BYUH:

1. The listed data plan price does not include carrier surcharge fees and taxes. I understand that these additional charges will be applied to me.
2. My plan includes up to 10GB of highspeed.
3. I understand there is a $30.0 non-refundable setup/activation fee for new lines and I will be charged this fee after the line has been setup.
4. I understand that BYUH Cellphones does not have the technical expertise to set up my tablet or watch beyond configuring the cellular connection.
5. I acknowledge that if I have any questions about this form, it is my responsibility to contact the appropriate office before submitting the form. I can do this by emailing cellphone@byuh.edu or calling (808) 675-3569.

Billing for BYUH Employees:
Setup: You agree to set up Payroll Deduction to pay for the monthly data plan and miscellaneous charges for all line you select.

Deduction Schedule:

• The total charges will be divided in half and deducted from each pay period during the month.
• For bi-weekly employees: There will be no deduction from the occasional third payday in the month.

Billing for BYUH Retirees and Alumni:

• Monthly Billing: As a retiree or alumni, you will be billed monthly for your data plan.
• Responsibility: You are also responsible for any lines you agree to set up.

Billing for BYUH Students:
Setup: You agree to charge your BYUH Student Account to pay for the monthly data plan by semester/term for all lines you select. Your plan will continue each semester/term until you submit a request to cancel your line.
