This form is for BYUH Employees/Retirees or PCC Employees that wish to setup a new line for cellular a watch.
-All new watch lines require a $10 activation fee
-Watch lines are charged $15/month +taxes and fees (estimated total: $21)

This watch will be tied to it's own dedicated phone with cellular service meaning that the user will have both a cellular line for their own watch and another line for their cellular watchThis watch will be a stand alone device, meaning that the user will not have a dedicated phone with cellular service, however, the watch may be connected to another user's phone such as a parent's phone, or to a phone without cellular service that only connected through wifi

Non-Refundable Activation Fee
- There is a $10.00 non-refundable activation fee for each "NEW" line setup with BYU–Hawaii.
Refunds or cancellations
The $10 activation fee is non-refundable. Plans are month to month and can be cancelled at any time. You will be charged a prorated rate depending on where in the month that you cancel your plan.
Payment
BYUH employees will have their monthly payment deducted from their paycheck. BYUH retirees and PCC employees will be invoiced each month. It is the responsibility of the BYUH retiree or PCC employee to make sure invoices are paid by the payment due date to avoid a line suspension.
